1. Participates as geriatric assessor in inter-professional Comprehensive Geriatric Assessment (CGA):
- Completes clinical triage for new referrals
- Data gathering and recording within all domains of Comprehensive Geriatric Assessment (CGA)
- Identification of patient/family goals and coordinated care planning. Patient and family interview utilizing strength-based and person-centred approach
- Implements care plans in collaboration with patient and caregiver/s to maximize independence at the highest level possible
2. Provide Clinical Care and Consultation Services to improve and/or maintain activities of daily living for frail older adults and seniors.:
- Analyze frail older adults and seniors’ capabilities and expectations related to life activities through observation, interviews and formal assessments.
- In collaboration with the GAIN inter-professional team, establish care plans for patients
- Develop intervention programs to address patients’ functional capacity related to self-care, productivity and leisure activities, and home safety.
- Consult and advise on health promotion programs to prevent disabilities and to maximize independent function in all activities of life
- Collaborate with GAIN partners including other GAIN teams and community resources to consult about patients and to carry out interventions (i.e.. home safety, mobility aids, cognitive interventions for safety in the home, assistive technology etc.)
- Educate patients and/or their caregivers to promote functional independence and enable the patient to achieve and maintain optimal health outcomes. :
- Staying up-to-date on relevant literature to assist team in choosing and utilizing appropriate cognitive assessment tools based on need. Conducting specialized cognitive assessment as required.

Qualifications:
Education::
- University Degree in Occupational Therapy
- Must be registered and in good standing with the College of Occupational Therapists of Ontario
- Registration with CAOT (Canadian Association of Occupational Therapists) or OSOT (Ontario Society of Occupational Therapists) an asset
- Current CPR Certification
Skills & Experience:
- Demonstrated extensive knowledge and skill working with individuals with various types of dementia, delirium, mental health, addictions, responsive behaviours, other neurological conditions, and those who experience an acute behavioural change
- Experience and knowledge of working therapeutically with geriatric populations an asset
- Demonstrated sound knowledge of cognitive and mobility assessment tools
- Certification in Gentle Persuasive Approaches, PIECES or Montessori preferred
- Skilled at crisis intervention and assessment preferred
- Initiative and adaptability skills
- Excellent communication skills (written and verbal) in English
- Proficiency in other languages desired (Armenian, Greek, Mandarin, Tamil, Hindi, Urdu)
- Access to a vehicle and valid driver’s license
Working Conditions:
Required to conduct home visits with patients and caregivers; working alone in the home
Required to work occasional evenings and weekends as required
- Regular interruptions to support patients’ needs and crises
Regular exposure to computer terminal
May be exposed to communicable diseases
Hours of Work:
- 35 hours work per week

- Criminal Reference Check required
A valid driver’s license and a car is required for home visits
